ABSTRACT

Rabdosia serra(R.serra),an important component of Chinese herbal tea,has traditionally been used to treat hepatitis,jaundice,cholecystitis,and colitis.However,the chemical composition of R.serra and its effect against colitis remain unclear.In this study,the chemical composition of the water extract of R.serra was analyzed using ultra performance liquid chromatography coupled with a hybrid linear ion trap quadrupole-orbitrap mass spectrometer(UPLC-LTQ-Orbitrap-MS).A total of 46 compounds,comprising ent-kaurane diterpenoids,flavonoids,phenolic acids,and steroids,were identified in the water extract of R.serra,and the extract could significantly alleviate dextran sulfate sodium salt-induced colitis by improving colon length,upregulating anti-inflammatory factors,downregulating proinflammatory fac-tors,and restoring the balance of T helper 17/T regulatory cells.R.serra also preserved intestinal barrier function by increasing the level of tight junction proteins(zonula occludens 1 and occludin)in mouse colonic tissue.In addition,R.serra modulated the gut microbiota composition by increasing bacterial richness and diversity,increasing the abundance of beneficial bacteria(Muribaculaceae,Bacteroides,Lactobacillus,and Prevotellaceae_UCG-O01),and decreasing the abundance of pathogenic bacteria(Turi-cibacter,Eubacterium_fissicatena_group,and Eubacterium_xylanophilum_group).Gut microbiota depletion by antibiotics further confirmed that R.serra alleviated colitis in a microbiota-dependent manner.Overall,our findings provide chemical and biological evidence for the potential application of R.serra in the management of colitis.

ABSTRACT

Objective:To investigate the changes of intestinal microecology in the early stage of sepsis rat model by 16S rDNA sequencing.Methods:Sixty male Sprague-Dawley (SD) rats were randomly divided into cecal ligation and puncture (CLP) group and sham operation group (Sham group), with 30 rats in each group. In the CLP group, sepsis rat model was reproduced by CLP method; the rats in the Sham group only underwent laparotomy without CLP. At 24 hours after the operation, the intestinal feces and serum samples of 8 rats in each group were collected. The survival rate of the rest rats was observed until the 7th day. The level of serum tumor necrosis factor-α (TNF-α) was detected by enzyme-linked immunosorbent assay (ELISA). Intestinal feces were sequenced by 16S rDNA sequencing technology. The operational taxonomic unit (OTU) data obtained after sequence comparison and clustering was used for α diversity and β diversity analysis, principal coordinate analysis and linear discriminant analysis effect size analysis (LEfSe) to observe the changes of intestinal microecology in early sepsis rats and excavate the marker flora.Results:At 24 hours after the reproduction of the model, the rats in the CLP group showed shortness of breath, scattered hair and other manifestations, and the level of serum TNF-α increased significantly as compared with that in the Sham group (ng/L: 43.95±9.05 vs. 11.08±3.27, P < 0.01). On the 7th day after modeling, the cumulative survival rate of the Sham group was 100%, while that of the CLP group was 31.82%. Diversity analysis showed that there was no significant difference in α diversity parameter between the Sham group and the CLP group (number of species: 520.00±52.15 vs. 492.25±86.61, Chao1 richness estimator: 707.25±65.69 vs. 668.93±96.50, Shannon index: 5.74±0.42 vs. 5.79±0.91, Simpson index: 0.93±0.03 vs. 0.94±0.05, all P > 0.05). However, the β diversity analysis showed that the difference between groups was greater than that within groups whether weighted according to OTU or not (abundance weighted matrix: R = 0.23, P = 0.04; abundance unweighted matrix: R = 0.32, P = 0.01). At the phylum level, the abundance of Proteobacteria and Candidatus_sacchari in the CLP group increased significantly as compared with the Sham group [18.100% (15.271%, 26.665%) vs. 6.974% (2.854%, 9.764%), 0.125% (0.027%, 0.159%)% vs. 0.018% (0.008%, 0.021%), both P < 0.05]. At the genus level, the abundance of opportunistic pathogen including Helicobacter, Ruthenium, Streptococcus, Clostridium ⅩⅧ in the CLP group was significantly higher than that in the Sham group [5.090% (1.812%, 6.598%) vs. 0.083% (0.034%, 0.198%), 0.244% (0.116%, 0.330%) vs. 0.016% (0.008%, 0.029%), 0.006% (0.003%, 0.010%) vs. 0.001% (0%, 0.003%), 0.094% (0.035%, 0.430%) vs. 0.007% (0.003%, 0.030%), all P < 0.05], and the abundance of probiotics such as Alloprevotella and Romboustia was significantly lower than that in the Sham group [7.345% (3.662%, 11.546%) vs. 22.504% (14.403%, 26.928%), 0.113% (0.047%, 0.196%) vs. 1.229% (0.809%, 2.29%), both P < 0.01]. LEfSe analysis showed that the probiotics belonging to Firmicutes were significantly enriched in the Sham group, and Romboustia was the most significantly enriched species. Opportunistic pathogens such as Helicobacter, Streptococcus and Clostridium ⅩⅧ were significantly enriched in the CLP group, Helicobacter_NGSU_ 2015 was the most significantly enriched species. Conclusion:In the early stage of sepsis, the intestinal microbiota structure of rats is significantly changed, which mainly shows that the abundance of Alloprevotella and other probiotics is significantly reduced, while that of Helicobacter and other opportunistic pathogens is significantly increased.

ABSTRACT

Objective:To investigate the transcriptomic mechanisms and clinical efficacy of D-CAG regimen for the treatment of acute myeloid leukemia (AML) after failure to initial induction of remission.Methods:The transcriptome data of AML cells before and after the use of dexitabine before August 28, 2021 was searched in Gene Expression Omnibus (GEO) database with "decitabine" as the search term. The R language package was used for differential expression analysis and Kyoto encyclopedia of genes and genomes (KEGG) and gene ontology (GO) enrichment analysis of the data. Protein-protein interaction network (PPI) analysis was conducted on the STRING online analysis website. The accurate treatment prediction platform designed based on logistic omics theory (EpiMed) was used to make drug-disease-target correlation analysis. The clinical data of 18 AML patients treated with D-CAG regimen after failure to induction of remission with standard anthracycline and cytarabine regimen ("3+7" regimen) in the 305th Hospital of Chinese PLA from October 8, 2015 to July 9, 2018 were searched and analyzed, and the curative effect was evaluated. The effects of the dose and duration of each drug on the efficacy were analyzed.Results:The transcriptome data of AML cells before and after the use of decitabine in GSE40442 dataset of the GPL5188 platform were finally selected, updated on July 10, 2014. A total of 366 differentially expressed genes were screened, including 201 up-regulated genes and 165 down-regulated genes. The differential genes were mainly related to cell cycle regulation, bone marrow leukocyte migration and differentiation, transcriptional regulation, bone marrow hematopoiesis and other signaling pathways. Ten core genes such as ANXA5, IL-10, THBS1, TLR4, JUN and CXCL12 were screened by PPI analysis. Drug-disease-target analysis showed that dexitabine had a potential therapeutic effect on various blood diseases such as diffuse large B-cell lymphoma, thrombocytopenia, T-cell acute lymphocytic leukemia, aplastic anemia, and AML. Of the 18 patients, after initial induction of remission, 7 (38.8%) patients achieved partial remission (PR), and 11 (61.2%) patients had no response (NR); after one cycle of re-induction remission therapy, 9 patients had complete remission (CR), 5 patients had PR, 4 patients had NR, and the overall response rate (ORR) was 77.8% (14/18). Compared with patients with NR, the CR rate was higher in patients with PR after initial induction therapy, which were 85.7% (6/7) and 27.3% (3/11), and the difference was statistically significant ( χ2=5.84, P = 0.025). The median duration of cytarabine in CR patients was longer than that in NR patients [10 d (7-14 d) vs. 5 d (2-8 d), Z = 3.89, P = 0.002] and the median ratio of the number of bone marrow blast cells to the duration cytarabine was lower in CR patients than that in NR patients [2.29 (0.89-9.10) vs. 8.10 (3.00-38.50), Z = -2.19, P = 0.006]; the median dose of cytarabine in CR patients was lower than NR patients, which were 50 mg·m -2·d -1 (30-150 mg·m -2·d -1) and 100 mg·m -2·d -1 (50-500 mg·m -2·d -1), and the difference was not statistically significant ( Z = -1.80, P = 0.074). Conclusions:AML patients with PR after initial induction and failure to initial induction of remission may be more likely to achieve CR after the treatment of D-CAG regimen, and this change may be related to the epigenetic regulation of decitabine.

ABSTRACT

ObjectiveTo analyze the feasibility, reliability and validity of the disability assessment for dementia scale for the elderly. MethodsA total of 290 dementia patients from 17 survey sites in 13 districts and counties of Chengdu were enrolled by convenient sampling method, and they were assessed using demographic data inventory, Mini-Mental State Examination (MMSE), Neuropsychiatric Inventory (NPI), Physical Self-Maintenance Scale (PSMS) and disability assessment for dementia scale for the elderly. Cronbach's α coefficient, Spearman-Brown coefficient, Guttman split-half coefficient and test-retest reliability were used to evaluate the internal consistency of above scales. Criterion-related validity of the scale was analyzed based on MMSE, NPI and PSMS. ResultsA total of 276 patients (95.17%) completed valid questionnaires. The intraclass correlation coefficient of the total score and each dimension were between 0.828~0.976, the Spearman-Brown coefficient were between 0.790~0.917, the Guttman split-half coefficient were between 0.812~0.857, and the Cronbach’s α coefficient were between 0.737~0.886. The cognitive function dimension score was positively correlated with the MMSE score (r=0.948, P<0.01), the mental behavior symptom dimension score was positively correlated with the NPI score (r=0.893, P<0.01), and the daily living ability dimension score was positively correlated with the PSMS score (r=0.997, P<0.01). The dimensions scores were positively correlated with the total score of the scale (r=0.634~0.841, P<0.05). ConclusionDisability of dementia assessment scale has good feasibility, reliability and validity, which is a reliable tool to assess senile dementia and disability.

ABSTRACT

Due to the rapid proliferation and growth of tumor cells, hypoxic microenvironment exists in many solid tumors, and hypoxia inducible factors (HIF) are critical for sensing oxygen tension within tumors and subsequently mediating the activation of hypoxia responses. Hepatocellular carcinoma (HCC) is one of the most hypoxic tumors. This article summarizes the mechanism of action of HIF in promoting the development and progression of HCC by promoting glycolysis, angiogenesis, invasion and metastasis, immune escape, and cancer stem cell formation. At present, the development of targeted drugs for HIF inhibitors has broad prospects in the treatment of HCC, and the detection of HIF also has a potential value in prognostic evaluation HCC.

ABSTRACT

Objective:To observe the drainage effects of the venous and arterial perivascular and adventitial connective tissues(PACT)on edema of lower limbs in mice model, and to provide a new understanding of the drainage for the elderly lower extremity edema.Methods:Different doses of fluorescent sodium(FluoNa)were injected into the venous and arterial perivascular connective tissues in the feet and ankles of the lower limbs to create a model of lower extremity edema in mice.Then, at the level of the femoral artery and vein sheath, a stereoscopic fluorescence microscope was used to observe and record the drainage effect of vascular PACT pathways on the edema.Results:By tracking the diffusion and flowing of FluoNa in the interstitial space of ankle and foot, we found that(1)in addition to the local diffusion at the injection site, FluoNa can also flow along the venous and arterial PACT and the interstitial spaces between the corresponding vein and artery.(2)The flowing process along the vascular adventitia included transmission along the vascular long axis, and the long axis-perpendicular diffusion into the vascular peripheral tissue.(3)When low-dose and high-dose FluoNa were administered into the venous adventitia of the lower extremities, the venous PACT imaging was observed.(4)When low-dose and high-dose FluoNa were administered into the arterial adventitia of the lower extremities, the arterial PACT imaging was observed; (5)When low-dose and high-dose of FluoNa were administered into the near the arteriovenous-accompanying blood vessels, not only the venous PACT pathway and the arterial PACT pathway are imaged, but also the interstitial space between the pathway of the accompanying vein and artery was also visualized at the same time; (6)Histological analysis showed that the tissues stained by FluoNa were the venous and arterial adventitia and perivascular connective tissues.Conclusions:The interstitial fluid in lower extremity tissue edema can not only be drained by the venous or arterial PACT pathway, but also be drained through the interstitial space between the accompanying vein and artery when a large amount of the interstitial fluid accumulates.

ABSTRACT

Objective:To analyze of hot spots and cutting-edge dynamic researches on Traditional Chinese Medicine (TCM) treatment for atopic dermatitis (AD).Methods:The data were abstracted from the literature of AD from CNKI, VIP and WanFang database, during January 1st, 2000 to May 31st, 2021. CiteSpace was used to map Knowledge and visualize information on the law, research hot spots and trendency of TCM in the treatment of atopic dermatitis in recent 20 years.Results:A total of 478 articles were included, mainly from TCM colleges and universities in various provinces. Most of the cooperative institutions have subsidiary relationships, and there is less cross-agency and cross-regional cooperation. Hot research topics included AD, clinical research, TCM therapy, TCM syndrome differentiation, compound Glycyrrhizin, TCM external treatment, and TCM formular, forming 8 main clusters. Integrated Traditional Chinese and Western Medicine was the developing trend in the treatment of AD, and the establishment of animal models to evaluate the therapeutic effect is the research frontier.Conclusion:Knowledge mapping can show the evolution path, research hotspot and frontier of TCM in the treatment of AD, and lay a foundation for high quality researches.

ABSTRACT

The report elaborated a case of a 52-year-old female patients with over 10 years of schizophrenia complicating 12 months of stupor. After a course of modified electroconvulsive therapy(MECT) combined with risperidone oral solution, the patient gradually became active and functionally recovered, and remained stable. This case suggests that MECT combined with risperidone may allow schizophrenia patients in a state of prolonged stupor to obtain clinical rehabilitation and effectively stabilize psychotic symptoms. The discussion is based on this case with a view to providing references for clinical treatment.

ABSTRACT

Interstitial fluid (ISF) flow through vascular adventitia has been discovered recently. However, its kinetic pattern was unclear. We used histological and topographical identification to observe ISF flow along venous vessels in rabbits. By magnetic resonance imaging (MRI) in live subjects, the inherent pathways of ISF flow from the ankle dermis through the legs, abdomen, and thorax were enhanced by paramagnetic contrast. By fluorescence stereomicroscopy and layer-by-layer dissection after the rabbits were sacrificed, the perivascular and adventitial connective tissues (PACTs) along the saphenous veins and inferior vena cava were found to be stained by sodium fluorescein from the ankle dermis, which coincided with the findings by MRI. The direction of ISF transport in a venous PACT pathway was the same as that of venous blood flow. By confocal microscopy and histological analysis, the stained PACT pathways were verified to be the fibrous connective tissues, consisting of longitudinally assembled fibers. Real-time observations by fluorescence stereomicroscopy revealed at least two types of spaces for ISF flow: one along adventitial fibers and another one between the vascular adventitia and its covering fascia. Using nanoparticles and surfactants, a PACT pathway was found to be accessible by a nanoparticle of <100 nm and contained two parts: a transport channel and an absorptive part. The calculated velocity of continuous ISF flow along fibers of the PACT pathway was 3.6‒15.6 mm/s. These data revealed that a PACT pathway was a "slit-shaped" porous biomaterial, comprising a longitudinal transport channel and an absorptive part for imbibition. The use of surfactants suggested that interfacial tension might play an essential role in layers of continuous ISF flow along vascular vessels. A hypothetical "gel pump" is proposed based on interfacial tension and interactions to regulate ISF flow. These experimental findings may inspire future studies to explore the physiological and pathophysiological functions of vascular ISF or interfacial fluid flow among interstitial connective tissues throughout the body.

Objective@#To evaluate the efficacy and safety of Oryz-Aspergillus enzyme and pancreatin tablets (Combizym®) in the treatment of postprandial distress syndrome (PDS) in the elderly, compared with gastrointestinal motility drugs.@*Methods@#A prospective randomized controlled trial was designed and registered in the China Clinical Trials Registry (ChiCTR-IPR-16008185). The elderly patients with PDS were randomly divided into three groups, including Mosapride group with Mosapride citrate tablets 5 mg 3 times per day for 2 weeks; Combizym® group with Combizym tablets 244 mg 3 times per day for 2 weeks; combined treatment group with both drugs and same doses for 2 weeks. The modified Nepean dyspepsia index (NDSI) score, discomfort intensity score and PDS score were calculated on patients before treatment, at the end of first and second week of treatment, as well as 4 weeks after treatment finished, respectively. Adverse effects were evaluated.@*Results@#A total of 323 patients from 16 tertiary hospitals in China were enrolled in this study. Among them, 105 patients were in Mosapride group, 109 in Combizym® group and 109 in combined treatment group. There were 148 males (45.8%) and 175 females (54.2%) with median age 71.4±9.0 years (60-100 years). Baseline characteristics of three groups were comparable. After treatment, the NDSI scores in three groups all decreased significantly (P<0.001), while they were similar between groups (P>0.05). The discomfort intensity score and PDS score in three groups showed a significant reduction after treatment (P<0.001), especially in the combined treatment group. Compared with Mosapride group, the scores in Combizym® group decreased significantly after one or two weeks [discomfort intensity score: after one week, 4.0(2.5, 8.0) vs. 6.0(3.0, 10.0); after two weeks, 3.0(0.0, 5.0) vs. 4.0(2.0, 6.0); all P<0.05. PDS score: after one week, 6.0(3.0, 9.0) vs. 7.0(3.5, 10.5); after two weeks, 3.0(0.0, 5.0) vs. 4.0(2.0, 7.0); all P<0.05]. The efficacy rate in all patients after first week of treatment was over 15.0%. The efficacy rates after two weeks were 55.2%, 68.8% and 73.4% in Mosapride group, Combizym® group and combined treatment group, respectively. After two week treatment, the efficacy rates in Combizym® group (P=0.041) and combined group (P=0.006) were higher than that of Mosapride group. The recurrence rate of Mosapride group was 9.5%, which was significantly higher than that of Combizym® group (1.8%, P<0.05) and combined treatment group (1.8%, P<0.05). There were no serious adverse effects in the three groups.@*Conclusions@#The efficacy of Oryz-Aspergillus enzyme and pancreatin tablets is comparable with that of Mosapride in elderly PDS patients, with fewer adverse effects and low recurrence rate. Combination regimen indicates better efficacy than that of Oryz-Aspergillus enzyme and pancreatin tablets or Mosapride alone.

Objective@#To evaluate the impact of long-term treatment with Entecavir(ETV)on renal function in elderly patients with chronic hepatitis B(CHB).@*Methods@#Elderly patients with CHB receiving ETV(0.5 mg/d)antiviral therapy who were followed up for 234 weeks in Beijing Friendship Hospital were retrospective analyzed.The estimated glomerular filtration rate(eGFR)(CKD-EPI creatinine formula)was used to evaluate renal function changes at baseline and 52, 104, 156, 208 and 234 weeks after treatment.@*Results@#A total of 40 included patients with CHB aged 60-81 years were enrolled, with baseline eGFR value(n=40) of (64.25±11.32)ml·min-1·1.73 m-2.The eGFR values at all time points after treatment were as following: (63.85±11.53)ml·min-1·1.73 m-2, (63.71±11.58)ml·min-1·1.73 m-2, (59.95±9.59)ml·min-1·1.73 m-2, (62.28±15.32)ml·min-1·1.73 m-2, (65.78±15.99)ml·min-1·1.73 m-2 at 52 weeks(40 cases), 104 weeks(40 cases), 156 weeks(38 cases), 208 weeks(36 cases)and 234 weeks(28 cases)after ETV treatment respectively.And the difference of eGFR values among all time points was not statistically significant(F=0.291, P=0.072). The results of the pairwise comparison showed that as compared with the baseline eGFR value, the eGFR values had no statistically significant difference at all time points after treatment(P>0.05). The baseline creatinine value in all 40 patients was(57.23±9.67)μmol/L.Creatinine values at 52 weeks(15 cases), 104 weeks(5 cases), 156 weeks(6 cases), 208 weeks(8 cases)and 234 weeks(6 cases)after ETV treatment, were(63.78±8.63)μmol/L, (67.86±11.21)μmol/L, (71.46±13.75)μmol/L, (68.22±8.99)μmol/L and(79.53±9.58)μmol/L respectively, with no significant difference(F=0.266, P=0.067). ABSTRACT

Objective To evaluate the impact of long-term treatment with Entecavir(ETV)on renal function in elderly patients with chronic hepatitis B(CHB).Methods Elderly patients with CHB receiving ETV(0.5 mg/d)antiviral therapy who were followed up for 234 weeks in Beijing Friendship Hospital were retrospective analyzed.The estimated glomerular filtration rate (eGFR) (CKD-EPI creatinine formula)was used to evaluate renal function changes at baseline and 52,104,156,208 and 234 weeks after treatment.Results A total of 40 included patients with CHB aged 60-81 years were enrolled,with baseline eGFR value(n=40) of (64.25±11.32)ml · min-1 · 1.73 m-2.The eGFR values at all time points after treatment were as following..(63.85 ±11.53) ml · min-1 · 1.73 m-2,(63.71± 11.58)ml· min-1 · 1.73 m-2,(59.95±9.59)ml· min-1 · 1.73 m-2,(62.28± 15.32)ml· min-1 · 1.73 m-2,(65.78± 15.99)ml · min-1 · 1.73 m-2 at 52 weeks(40 cases),104 weeks(40 cases),156 weeks(38 cases),208 weeks(36 cases)and 234 weeks(28 cases)after ETV treatment respectively.And the difference of eGFR values among all time points was not statistically significant(F =0.291,P =0.072).The results of the pairwise comparison showed that as compared with the baseline eGFR value,the eGFR values had no statistically significant difference at all time points after treatment(P > 0.05).The baseline creatinine value in all 40 patients was (57.23 ± 9.67) μmol/L.Creatinine values at 52 weeks(15 cases),104 weeks(5 cases),156 weeks(6 cases),208 weeks (8 cases)and 234 weeks(6 cases) after ETV treatment,were(63.78 ± 8.63)μmol/L,(67.86 ± 11.21) μmol/L,(71.46 ± 13.75) μmol/L,(68.22 ± 8.99) μmol/L and (79.53 ± 9.58)μmol/L respectively,with no significant difference(F =0.266,P =0.067).The results of the pairwise comparison showed that as compared with the baseline creatinine value,the creatinine values had no significant difference at all time points after ETV treatment(P>0.05).Conclusions ETV treatment has no effect on renal function in elderly patients with chronic hepatitis B.

ABSTRACT

Objective To investigate the effects of methylation inhibitor 5-Aza-2'-deoxycytidine (5-Aza-dC) on biological behavior of esophageal squamous carcinoma cell (ESCC) lines KYSE140 and KYSE150. Methods KYSE140 and KYSE150 cell lines were divided into the blank group, the control group and the experimental group. The cells in the blank group didn't do the treatment, and the cells in the control group were added to DMSO 2 μmol/L, while in the experimental group, cells were treated with different concentration (1, 2, 3 and 4 μmol/L) of 5-Aza-dC which affected respectively at different time (24, 48, 72 and 96 h). Cell proliferation was detected by using methyl thiazolyl tetrazolium (MTT) assay and the optimal drug concentration and time point were selected. Transwell assay was performed to detect the change of cell migration and invasion. Flow cytometry was used to observe the effects of drugs on cell apoptosis and cell cycle.The expression of PARP,Caspase-3,CCNB-1,and CCNE-1 were detected by Western blot. Results MTT result showed that the effective function time of 5-Aza-dC on KYSE140 and KYSE150 was 96 h at the concentration of 4 μmol/L. Under this condition, the cell ability of migration and invasion was decreased significantly. The migrated cell number of KYSE140 and KYSE150 respectively in the blank group, the control group and the experimental group was (193.3±8.6), (184.0±10.4), (61.7±7.1) and (112.0±6.4), (101.3± 7.9), (26.3±5.7). The invasive cell number was (47.3±7.3), (38.7±5.1), (8.0±3.9) and (83.3±6.8), (74.7±5.7), (21.0±2.7), respectively. The difference was statistically significant (P <0.05). Flow cytometry revealed that 5-Aza-dC increased the apoptosis of KYSE140 and KYSE150. The apoptosis rate of the blank group, the control group and the experimental group was (2.8±0.3) %, (11.2±0.7) %, (18.6±0.6) % for KYSE140 and (2.7±0.4)%,(9.8±0.4)%,(17.7±0.5)% for KYSE150.Compared with the other two groups,the cell number of G2/M phase in the experimental group was increased remarkably (P < 0.05). PARP and Caspase-3 were sheared evidently and the protein expression of CCNB-1 was up-regulated while the expression of CCNE-1 was down-regulated in the experimental group. Conclusion 5-Aza-dC can inhibit the proliferation and promote apoptosis of ESCC cells.

ABSTRACT

Objective To analyze the clinical characteristics of hospital-acquired pneumonia in bedridden patients aged 90 years and over.Methods From January 1,2015 to August 1,2015,40 bedridden elderly patients(≥90 years)with hospital-acquired pneumonia admitted to Beijing Hospital were assigned into the observation group,with 40 non-bedridden elderly patients (≥ 90 years)with community acquired pneumonia serving as the control group.Basic clinical data,infection symptoms and inflammation markers,and changes in serological test results before and after pneumonia were compared between the groups.Results Compared with the control group,the observation group had much lower body mass index(20.7± 1.2)kg/m2 vs.(25.1 ± 3.1) kg/m2 (t=-8.43,P<0.01),and was accompanied with more diseases and associated with more symptoms during infection (all P< 0.05).Meanwhile,patients with lung infections in the observation group were more likely to have atrial fibrillation,often with increased levels of brain natriuretic peptide(1 047.9 ± 193.5) pg/L vs.(3 387.8± 763.8) μg/L (t =-3.39,P<0.01) and abnormalities in troponin,but without obvious elevations of inflammatory markers such as white blood cells,C reactive protein and procaleitonin.Conclusions Bedridden patients aged 90 years and over with hospital-acquired pneumonia may show atypical clinical presentations and increased accompanying symptoms.Comprehensive geriatric assessment is critical for good prognosis.

ABSTRACT

Objective To evaluate the opening level and optimal time window of the blood-brain barrier induced by adenosine A2 receptor agonist ( Lexiscan) via dynamic enhanced MRI. Methods Twen-ty New Zealand white rabbits were divided into experiment group ( group A, n=10) and control group ( group B, n=10) . Rabbits in group A were injected with Lexiscan and rabbits in group B were injected with physiological salt via ear vein, then the coronary scanning was performed. Contrast enhanced MRI was performed at different time points ( 5, 10, 15, 20 min, and then every 10 min, until 2 h) following the in-fusion of Gd-diethylene triamine pentaacetic acid (DTPA). The signal intensity (SI) of region of interest ( ROI) was measured and the percent enhancement of SI was calculated. Evens blue staining results in brain tissues were observed. Pair t test was used to analyze the data. Results The percent enhancement of SI in group A significantly increased to (40. 93±3.70)% at 5 min, reached the maximum of (43.03±3.62)% at 30 min, slowly decreased until 50 min, and got to a stable level at almost 80 min. At each time point, the per-cent enhancement of SI in group A was significantly higher than that in group B ( t values:6.88-20.28, all P<0. 05) . The staining was evident in group A. Conclusions Lexiscan can open blood-brain barrier tem-porarily and reversibly, and the optimal opening time window is 10-50 min post-injection.

ABSTRACT

[Objective]To comprehensively compare the feasibility of three different treatment strategies consisting of low-dose chemotherapy(LDC),surgery and surgery with adjuvant low-dose chemotherapy(SLDC)for children with solitary bone lesions of eosinophilic granuloma(SBL-EG).[Methods]We retrospectively reviewed the records of 149 pediatric patients with SBL-EG at our institutions from 2002 to 2014. Our study included 86 patients who received LDC ,33 patients who received surgery and 30 patients who received SLDC. The duration of hospital stay ,time to symptom relief,recovery time,cost,complications and relapse-free sur-vival(RFS)of each strategy were analyzed.[Results]Hospital stay,time to symptom relief,recovery time and cost in the LDC group were significantly shorter or less than those in the surgery or SLDC group (P 0.05). Chemotherapy-related adverse events in the LDC and SLDC groups included nausea(8.62%),aminotransferase elevation(7.76%),slight hair loss(4.31%), immunity decline (21.55%),growth retardation (10.34%) and moon face (7.76%). LDC and SLDC treatment resulted in a significantly longer RFS (147 months and 126 months ,respectively) than surgery alone (114 months)(P = 0.005 and 0.019 , respectively). However ,there was no statistically significant difference in RFS between the LDC and SLDC groups (P = 0.732).[Conclusions]Compared with surgery or SLDC,LDC appears to promote more rapid recovery,less invasion,increase safety and eco-nomic treatment strategy for pediatric patients with SBL-EG.

ABSTRACT

Objective To discuss the clinical effect of aspirin combined with clopidogrel therapy in pa?tients with transient ischemic attack( TIA)/light stroke. Methods One hundred and forty?eight cases with tran?sient ischemic attack/light stroke were divided randomly and voluntarily into the control group( n=74) and ob?servation group( n=74) . The patients of the control group were given aspirin based on conventional treatment while the observation group were given clopidogrel based on control group treatment. The serum C?reactive pro?tein(CRP),incidence of early neurologic deterioration(END),recurrence rate within 90 days,degree of nerve function defect score (NIHSS score),clinical effect after treatment and the complications during treatment be?tween the two groups were compared. Results After one week treatment,the serum CRP level of observation group decreased obviously compared with pre?treatment ( ( 6. 24 ± 1. 63 ) mg/L vs. ( 12. 03 ± 2. 46 ) mg/L , t=5. 625,P=0. 027),and the control group((9. 34±1. 24) mg/L,t=4. 348,P=0. 036),and there was not sig?nificantly reduced in the control group( t=2. 647,P=0. 186) . The incidence of END and recurrence rate within 90 days of observation group were evidently lower than that of control group(6. 76%(5/74) vs. 21. 62%(16/74) ,1. 35%( 1/74) vs. 8. 12%( 6/74);χ2=6. 737,5. 742,P=0. 022,0. 029) . The NIHSS score at 2 weeks af?ter treatment of observation group evidently lower than that of control group,the differences were statistically sig?nificant((2. 31±1. 03) points vs. (5. 34±1. 25) points;t=4. 663,4. 928). The effective rate of observation group was obviously higher than that of control group,the differences were statistically significant(93. 24% vs. 63. 51%,χ2=6. 253,P=0. 029) . There was no statistically significant differences about complications during treatment in both groups. Conclusion Aspirin combined with clopidogrel therapy can obviously improve nerve function,reduce progress of stroke and improve the clinical effect in patients with transient ischemic attack/light stroke,it is worth popularization and application.

ABSTRACT

Objective To explore the advantages and disadvantages of helicopter emergency medical services of South China in the long-distance transport for critical patients.Methods A total of 30 patients who received helicopter emergency medical services by Guangdong Generral Hospital from August 2004 to December 2014 were selected as the observation group,and the other 30 patients with similar conditions who received ground emergency medical services were selected as the control group.To analyses the difference between the two groups in the disease,transport distance,transportation time,costs and compliction by χ2-test,t-test and nonparametric test according types of data.Results There were significantly difference between two groups in transport distances (km) [578.0 (313.0,707.5)vs.214.5 (101.5,313.5),P 0.05).Conclusions Helicopter emergency medical services could shorten the transportation time of critical patients on long distance transportation,and improve the efficiency of first-aid.However,there were many disadvantages that need to be improved in the helicopter emergency medical service of China.

ABSTRACT

<p><b>OBJECTIVE</b>This study analyzed the alveolar changes of complete unilateral cleft lip (UCCL) patients after pre-surgical nasoalveolar molding (PNAM) treatment using a three-dimensional method.</p><p><b>METHODS</b>Palate impressions of thirty UCCL patients who underwent PNAM treatment in the Affiliated Stomatology Hospital of Guangxi Medical University were taken pre- and post-PNAM. The plaster impressions were scanned using cone beam computed tomography (CBCT). The DICOM files were exported and 3D models reconstructed in Mimics 15.0. The following parameters were analyzed in SPSS 17.0: cleft alveolar gap width, alveolar arch end width, upper lip frenulum-median plane distance, alveolar shift distance, and length of alveolar section.</p><p><b>RESULTS</b>After PNAM treatment, the cleft alveolar gap width and upper lip frenulum-median plane distance decreased significantly (P<0.05). No statistically significant change was observed in the alveolar arch end width (P>0.05). Alveolar shift distances for both sides increased significantly (P<0.05), and the shift distance of major alveolar sec-tion had a more significant increase than the minor alveolar section. The lengths of the alveolar section of both sides increased significantly (P<0.05) with the length of the major alveolar section increasing more significantly than the minor alveolar section.</p><p><b>CONCLUSIONS</b>The impression-CBCT method supply accurate three-dimensional data of the alveolar process. The PNAM treatment can significantly correct alveolar deformity and improve appearance.</p>
